HMO vs. PPO: which matters more in Tennessee?
Most Tennessee ACA plans are HMOs (Health Maintenance Organizations), which require you to use an in-network primary care provider and get referrals for specialists, in exchange for lower premiums. PPOs (Preferred Provider Organizations) cost more but allow more flexibility to see out-of-network providers without a referral.

In large metro areas like Nashville, Memphis, and Knoxville, HMO networks are usually broad enough that the lower premium makes sense for most people. In more rural counties with fewer contracted providers, a PPO's flexibility can be worth the extra cost.
Regional carrier strength
Carrier presence in Tennessee varies by region. Blue Cross Blue Shield of Tennessee, headquartered in Chattanooga, has the broadest statewide reach and is active in all 95 counties. Cigna Healthcare has strong urban presence around its Nashville-area footprint, while Ambetter and Oscar Health tend to concentrate coverage around Middle and East Tennessee's larger metros. UnitedHealthcare and Alliant Health Plans round out the market with more selective county-by-county participation. Checking your prescription coverage
Each carrier maintains its own drug formulary — the list of covered medications and what tier (and therefore what copay) they fall into. A medication covered cheaply on one carrier's formulary might sit on a much higher, more expensive tier with a different carrier, even at the same metal level. If you take regular prescriptions, checking the formulary before enrolling is one of the highest-value things you can do to avoid a surprise cost later.
On-exchange vs. off-exchange plans
Most carriers sell both "on-exchange" plans (available through HealthCare.gov, where premium tax credits can be applied) and "off-exchange" plans (bought directly from the carrier or through an agent outside the Marketplace). Off-exchange plans are never eligible for a premium tax credit, even if the plan itself looks identical to an on-exchange option, so unless you're certain you won't qualify for any subsidy, it's almost always worth starting your search on-exchange.
Matching a carrier to your health needs
Someone who's generally healthy and rarely visits a doctor may prioritize the lowest premium above all else, while someone managing an ongoing condition may find it worth paying more for a plan with a stronger specialist network or better prescription coverage for their specific medications. There's no single "best" carrier in Tennessee — the right one depends entirely on your own health situation, budget, and which providers you want to keep seeing.
